Gynecomastia
can be treated with either liposuction, using primarily
an ultrasonic or power assisted machine and/or with
a direct excision and liposuction. Both are performed
as an outpatient under general anesthesia by and
MD anesthesiologist.
I do prefer the liposuction approach for it involves
a very small scar placed on the side of the breast
but it becomes more difficult to extract all the
fat/breast tissue if there is more of the former.
Then, I would perform the direct excision, usually
through an incision around the areola about one
and half inches long. I then also use liposuction
to "feather" the edges so it is smoother.
Sometimes, I cannot determine what I end up doing
until the actual surgery. I first usually begin
with the liposuction.
However, if the patient wishes all the tissue gone,
I recommend the second approach.The approximate
cost of the liposuction alone with the ultrasonic
or power assisted machine is about $3500 while when
it is combined with the direct excision it is about
$6500.
The recovery is shorter for the former but I still
would recommend limited activity for the first two
weeks and usually full activity thereafter. Swelling
is present with both approaches and resolves over
several months. Once the tissue is removed (and
it is sent for pathological examination) it usually
does not recur unless there is a significant weight
gain. Since it is primarily cosmetic, the insurance
rarely covers the procedure, especially if liposuction
is involved.
Problems such as bleeding, fluid collection(sarcoma),
infection, unevenness, asymmetry and scarring may
occur but are minimal.
