CO2 LASER-The one I use most commonly for skin resurfacing is the ULTRAPULSE CO2 laser. This helps with texture changes of the face as well as fine lines. It can be done as a separate procedure or combined with other facial surgery.

It is usually performed as an outpatient under some sedation administered by an MD anesthesiologist. After safety precautions, the laser energy is directed at the skin surface. The CO2 vaporizes the epidermis layer by layer. After the pulse is applied, and it is emitted in a set pattern like a square, char results which is wiped off and then the laser is directed at the same area until an actual contraction of the skin is seen which I use as my endpoint.

I usually treat the entire face including the upper and lower eyelids as well as nose and down slightly into the neck. I cannot treat the neck or any other skin surfaces for it can cause scarring. After the treatment, I apply an ointment to the entire area which the patient continues to apply at home approximately five times per day for about five days until it heals.

Actually, it is very similar to healing from a burn which this, in effect, is, only controlled. After several days, it dries up and is a lobster red color which begins fading over the next week or so and continues fading over the succeeding four months on average. It goes from bright red to pink to either the person’s normal skin color or maybe even lighter-usually like an area that has never been exposed to the sun-the inner arm. It definitely removes the tan if present.

After it initially dries, the patient needs to use a strong sunscreen and avoid the sun with hats as much as possible until the redness goes away totally. After that, tanning can still occur but the patient’s skin is much more sensitive to the sun and its damaging effects.

It is not a pleasant process to go through, especially in the first week but usually a patient can be presentable after two weeks. During this first week, drainage,tightness,agitation, pain are common. Appropriate medication is prescribed and recommended! Swelling of the face is very normal also.

The results can be quite dramatic and most people are very pleased with them. It is especially suited for the sun worshipper and/or smoker and I treat a large number of men. It also has been shown to help with actinic changes as well as even basal cell cancers of the face.Its results are long lasting.

Complications due occur and the most significant is scarring which fortunately is rare. However, pigment changes like blotchiness or unevenness and a whitening can occur, especially if there is sun exposure before all the redness and pinkness disappears. These can be treated with additional procedures. My fee for facial resurfacing with the Ultrapulse CO2 laser is $6000(which includes the specific laser). The cost of the outpatient operating room and MD anesthesiologist is about $1300.
