Otoplasty
or the "pinning of the ears" is performed as an
outpatient under either intravenous sedation or
a general anesthetic administered by an anesthesiologist.
I remove a section of skin behind the ear and then
usually solely with sutures, reform the fold that
is missing as well as rotate the whole ear back
closer against the head and place stitches to keep
it in that position. The surgery takes about one
hour and I place a large, bulky dressing around
the whole head to protect the ear during healing.
The pain is normally not that great. The patient
needs to sleep sitting up and not on either ear
and is restricted from heavy activity at least two
weeks and light activity with the bandage is possible
within days.
I remove this bulky bandage about the fifth day
to make sure everything is healing well and re-apply
a bandage. In children, I keep this on for an additional
week, but in adults this can be shortened. After
this bandage is removed, there is still swelling,
sometimes black and blueness and tenderness. These
slowly resolve over time. Usually, though, they
are presentable at about ten days.
About 80% of the ear develops by the fourth year
and so this procedure can be done on children, usually
prior to kindergarten.
Since usually only sutures are used, a headband
is used over the ears to protect them during sleeping
for at least another two weeks. A stitch can break
if there is too much activity and the problem can
recur and this would need a further revision. Infection
and allergic reactions to the stitches occurs in
a minority of patients. The ears do come out some
after the initial two weeks and if there is any
difference prior to surgery, there usually is some
after.
EAR REDUCTION is much less common but can also be
performed in a similar fashion. This actually involves
the cutting out of a portion of the underlying cartilage
and overlying skin.
